April 28, 2009, the City Commission approved a Design/Build Agreement with D. Stephenson Construction, Inc., for the construction of the new Dania Beach Library in the amount of Three Million Nine Thousand Two Hundred Thirty Two Dollars ($3,009,232.00). The agreement anticipated and included the Site Plan, Building Plan and Elevations for the construction of the library; and modifications to include additional services that are needed to the Master Site Plan. Modifications resulted in Change Order Nos. 2, 3, 4, and 5. Approval of Change Orders Nos. 2, 3, 4 & 5. |

It is recommended that the City Commission approve the attached Change Order Nos. 2, 3, 4, and 5 to D. Stephenson, Inc. for a net increase of $96,614.00 in cost and NO time increase, under the terms and conditions and authorize the City Manager to execute same.
